The goods and services shown below are those we can provide to our customer. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our minimum services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain in writing on the statement we provide describing the funeral goods and services you selected.
BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF:
This charge includes, but is not limited to the staff and facilities necessary to respond to initial request for service, arrangement conference with the family, arrangement of funeral, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notices, staff assistance prior to, during and following the funeral, e.g. crematory, cemetery, and others.
This fee for our basic services and overhead is non declinable and will be added to the total cost of the arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving of remains.)
Except in certain special cases, embalming is not required by law. Embalming may be necessary if you select certain funeral arrangements such as a service with public viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

Our services include equipment and set up of visitation, use of the visitation room at our funeral home, placement of encased remains, display of floral arrangements, parking lot assistance, and supervision and attendance during the visitation.

B. Funeral Service at the Funeral Home or Another Facility

C. Memorial Service at the Funeral Home or Another Facility
Our services include coordinating the memorial service, set-up and use of equipment, supervision of the memorial service, and additional staff necessary to conduct memorial service.
D. Staff and Equipment To Conduct A Graveside Service
Our services include coordinating the funeral arrangement at the graveside, supervision of the graveside service, and equipment and staff necessary to conduct the graveside service.
E. Cemetery Set-up Fee When No Outer Burial Container Is Chosen
Our services include the cemetery set-up of a tent, grass, chairs, and lowering device to safely lower the casket into the grave. This fee is already included when your purchase a grave liner or vault.
F. Crematory Fee
This fee is the cremation fee that is charged by the crematory. This fee is already included in the charge for Direct Cremation.
G. Insurance Assignment and Processing Fee
10% of the total contract amount

BASIC DIRECT CREMATION:
The charge for a direct cremation includes the initial transfer of remains to the funeral home, local transportation to the crematory, necessary authorizations, basic services of the staff, and the cremation fee. If you want to arrange a direct cremation, you can use an alternative container. This charge includes our basic services of funeral director and staff, the transfer of the remains to the funeral home (within 50 Miles), and transportation to the cemetery (within 50 miles). This charge does not include visitation, rites, ceremonies, cemetery charges or cash advance items.
FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME
This charge includes the local transfer of remains to the funeral home, basic services of funeral director and staff, embalming, necessary authorizations, and local transportation to the airport and minimum shipping container. This charge does not include visitation, rites or ceremonies prior to forwarding of the body
