  1. We must have a signed cremation authorization form signed by the legal next of kin (call the funeral home if you have any questions).
  2. You must sign an itemized statement of funeral goods and services (this is the funeral bill completed by the funeral director).
  3. Payment must be made in full or secured by a life insurance policy.
